In order to determine appropriate accommodations and provide the most effective support services, students are required to submit documentation related to their disability. In most cases, documentation should be current and within the past three years. Documentation or evaluations must be completed and provided by a qualified professional.

Sufficient documentation must include:

  • Documentation needs to be dated within the last three years
  • Clearly stated that there is a diagnosed disability
  • Description of functional limitations resulting from disability
  • The accommodation(s) requested are clearly defined and adequate support for each requested accommodation is provided in the documentation
  • All documentation submitted needs to have the office’s letterhead and a signature from a qualified evaluator

Examples of sufficient documentation:

  • Current IEP or 504 Plan with evaluations used to create the educational plan
  • Complete educational, developmental psychological, or medical documentation that includes scores/subset scores
  • Any medical history relevant to disability for which accommodations are being requested
  • A formal letter from a diagnosing provider stating a diagnosed disability, functional limitations experiences and recommendations for potential accommodations.

When submitting a request for accommodations, please include educational, psychological, and or medical evaluations administered to create a 504 plan or Individualized Education Program (IEP). Evaluations or medical information should be current and dated within the last three years. Please note, only submitting a 504 plan, IEP, or a letter from a qualifying doctor without a current evaluation will only be used to document that there is a diagnosed disability and will need additional documentation to determine accommodations.

Submit all documentation to Student Affairs in the David Student Union 3127, by filling out the Request for Services and Accommodations Form when requesting accommodations.

While we do not provide diagnostic testing for students, we are able to share information about local evaluation professionals and resources. Students may also pursue evaluations through qualified professionals within their school system or in their local community. Students are encouraged to plan accordingly as an evaluation process can take time to complete.

All information related to a student’s disability is kept confidential. Information will only be shared with faculty or staff members who have a legitimate educational or professional need to know.
